"Opening Doors To Independence"
Adult Foster Care
24 hour care in a non-medical facility for frail elderly, physically or emotionally disabled adults (18 years of age or older) that are on SSI or Old Age Pension (OAP).
Children's Home & Community Based Services Program (Model 200)
A waiver program for disabled children, birth through 17 years of age who are at risk of institutionalization in a hospital or nursing facility and who would not otherwise be eligible for Medicaid due to parental income and/or resources.
Home Care Allowance
A special non-entitlement allowance for the purpose of securing services to an individual of any age, in his/her home, based on the case manager's assessment. Eligibility for and authorized amounts of the HCA are subject to available appropriations.
Home & Community Based Services for the Brain Injured
Services provided in a home or community setting to individuals (16 to 64 years of age) meeting rehabilitation criteria and with cognitive deficits
  • who are eligible for Medicaid reimbursement post acute care,
  • who would require continued hospitalization without the provision of intensive HCBS-BI services, and
  • for whom HCBS-BI services can be provided at no more than the cost of hospital care.
Home & Community Based Services for Elderly, Blind and Disabled
Services provided in a home community setting to individuals (18 years of age or older)
  • who are eligible for Medicaid reimbursement for long term care,
  • who would require nursing facility care without the provision of HCBS-EBD, and
  • for whom HCBS-EBD services can be provided at no more than the cost of nursing facility care
Home & Community Based Services for Persons Living with AIDS
Services provided in a home or community setting to individuals, regardless of age
  • who are eligible for Medicaid reimbursement for long term care,
  • who would require nursing facility care or hospital care without the provision of HCBS-PLWA, and
  • for whom HCBS-PLWA services can be provided at
    • no more than the cost of nursing facility care for persons needing nursing facility level of care, or
    • no more than the cost of hospital care for persons needing hospital level of care.
Nursing Home Care
Part of the standard Medicaid benefit package under Long Term Care.
Private Care Management
Provides case management services to clients over 18 years of age based upon a mutual agreement to assist them in the assessment and coordination of their needs. Clients may choose a service package or on going services tailored to meet one's individual needs.
